PTo enroll in the driving school, you must submit a completed “Application for Admission to Training” as well as a completed and doctor-certified “Medical Certificate of Fitness to Drive Motor Vehicles.”

Both the application and the medical certificate are provided as fillable forms that you can complete before printing.
